The Supports Coordinator’s Role

Achieving More considers our supports coordinators the lynchpin of our supports team. They function as facilitators, implementers, and liaisons. They manage all discussions surrounding the best methods for resolving the needs of the individuals under their care. Supports coordination includes locating, scheduling, and monitoring services and supports waiver participants receive.

Supports coordinators assume critical roles in connecting people with intellectual or developmental disabilities with the services they are entitled to:

  • Identifying appropriate services and supports
  • Advocating for the best interests of the participant based on his or her needs, preferences, and goals

Supports coordinators assure compliance with Medicaid Waiver Requirements:

  • Monitoring participants’ the health and welfare
  • Ensuring individuals receive all services prescribed in the ISP as outlined in the Waivers and AE Operating Agreement
  • Maintaining current documentation

Their duties are varied and comprehensive:

  • Supports Intensity Scale & PAPlus assessments
  • Participant data collection through PUNS, the prioritization for urgency of need for services
  • Service notes and billable and non-billable service delivery documentation and maintenance
  • Incident management
  • Independent service quality monitoring
  • Due process for waiver and non-waiver funded services
  • Preferred service delivery
  • Home and Community Services Information System utilization
  • Individual support plan facilitation and completion
  • Documentation and follow-up
