Supports Coordination is a critical service that involves the primary functions of locating, coordinating, and monitoring needed services and supports for individuals. The most important element of quality Supports Coordination is building relationships. When strong relationships are developed, the quality of support and services improves. Building relationships is not a separate and distinct activity; it is integral to each function the supports coordinator performs. Locating services and supports consists of assistance to the individual and his or her family in linking, arranging for, and obtaining services specified in the support plan, including resources in the community, competitive integrated employment, needed medical, social, habilitation, and participant direction opportunities.
Who We Serve
- Individuals diagnosed with an intellectual disability (any age).
- Individuals diagnosed with autism (any age).
- Individuals diagnosed with a medically complex condition (under the age of 21).
How to Register
Contact the county where you reside and say that you would like to become qualified to receive Supports Coordination services. The county will work with you to determine your eligibility for Supports Coordination services. Once you are deemed eligible, the county will provide you with a list of Supports Coordination Organizations to choose from. Your supports coordinator will consult with you and your support team to outline a program to achieve your goals.
We will ensure ongoing support and your continued satisfaction.
We know you, your family, and your support team are best equipped to assess your ambitions and determine the achievements you aspire to. Your supports coordinator is your partner and collaborator. He or she will discuss these objectives with you and draft a written individual service plan (ISP) that will serve as the roadmap to the care and aid that will bring those goals within reach.
