When they engage with Achieving More, participants in Pennsylvania’s Consolidated Waiver, Community Living Waiver, and Person/Family Directed Support Waiver programs receive expert, personalized, and authentic service delivered with compassion and understanding.

We will arrange a meeting within 48 hours of initial outreach.

You will be assigned a personal supports coordinator.

Your supports coordinator will consult with you and your support team to outline a program to achieve your goals.

We will ensure ongoing support and your continued satisfaction.
We know you, your family, and your support team are best equipped to assess your ambitions and determine the achievements you aspire to. Your supports coordinator is your partner and collaborator. He or she will discuss these objectives with you and draft a written individual service plan (ISP) that will serve as the roadmap to the care and aid that will bring those goals within reach.

Achieving More works with individuals who qualify for assistance through one of Pennsylvania’s waiver programs and meet these level of care requirements:
- Undergo a medical evaluation
- Receive a diagnosis of an intellectual or developmental disability or autism
- Obtain a recommendation for an intermediate care facility level of care based on a medical evaluation
- Qualify for medical assistance
- Meet the financial requirements as determined by your local county assistance office.
Person/family Directed Support Waiver

- There is no age limit for individuals with intellectual disabilities or autism.

- Diagnosis of an Intellectual Disability, Autism, or Developmental Disability
- Recommended for an Intermediate Care Facility (ICF) Level of Care Based on a Medical Evaluation
- Determined Eligible for Medical Assistance (MA)
- Meet the Financial Requirements as Determined by Your Local County Assistance Office.
Individual cost limit of $41,000 per person per fiscal year
- Excludes supports coordination and supports broker services
- Can be exceeded by $15,000 for advanced supported employment or supported employment services
Community Living Waiver

Level of Care
- Medical Evaluation

What does it offer?
Supports individuals to live more independently in their homes and communities through the provision of a variety of services that promote community living, employment, communication, self-direction, choice, and control.
- Provide services to approximately 1,000 individuals with an intellectual disability or autism, as well as children under age 9 with a developmental disability who have a high probability of resulting in an intellectual disability or autism.
Consolidated Waiver
Age Restrictions
- Individuals of any age with an intellectual disability (ID) or autism.
- Children with a developmental disability under age 9 with a high probability of resulting in an ID or autism diagnosis.
- Children under age 22 with a developmental disability due to a medically complex condition.
- There Is No Age Limit for Individuals With an Intellectual Disability or Autism.
